Registration Form (Including Emergency Card for Fieldtrips and Walks)

This form must accompany your child to their gradual entry. Please note that this information is on file at the centre and contains important contact information. Should this information change, it is imperative that you make the necessary changes to your forms. This can be done by speaking with a classroom teacher or a Program Manager.
